  • Please complete this application online or mail the application with the contract, payment and health form (applies to food vendors only) to:

  • Application, contract & payment must be received by June 30, 2019.
  • You can pay online via credit card.
  • Money Orders or Bank Certified Checks should be made payable to: IBA
  • No personal or business checks will be accepted.
  • IBA will NOT be responsible for the security of your vending space or equipment.
